Health department conducts inspections

Mouse droppings were at a Northwood hotel during the last three Wood County Health Department inspections.

Inspectors visited Baymont Inn & Suites, 2426 Oregon Road, on March 14 and told the operators they were not to operate with open food products until the facility was treated by a licensed pest control operator, an invoice of treatment was obtained, and an improvement in the amount of pest droppings was observed throughout facility.

Inspectors observed mouse droppings on the prep table and sinks across from the grill line.

The hotel was told to contact the health department for a re-check once facility has been treated by licensed pest control operator. No other inspections have been recorded since March 14.

Mouse droppings also were found in the kitchen during inspections that were conducted March 8 and Feb. 8.

Six critical and nine non-critical violations were recorded at the most recent inspection.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health (repeat); equipment food-contact surfaces or utensils are unclean (repeat); presence of live insects, rodents, and other pests (repeat); food contact surfaces not easily cleanable (repeat); water under pressure is not available to all fixtures as required; and direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed.

Non-critical were no written procedures for responding to vomiting or diarrheal events (repeat); non-food contact surfaces not easily cleanable (repeat); cutting blocks or boards cannot be effectively cleaned and sanitized (repeat); equipment not approved by a recognized testing agency (repeat); no sanitizer test kit available (repeat); non-food contact surfaces of equipment are unclean (repeat); unnecessary or nonfunctional items and/or litter on premises (repeat); physical facilities not maintained in good repair (repeat); and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

The following inspections were done March 13.

Little Caesars Pizza, 1068 N. Main St., Bowling Green, had five non-critical violations, which were facility does not have an employee with manager certification in food protection; no written procedures for time as a public health control; non-food contact surfaces of equipment are unclean (repeat); physical facilities not maintained in good repair (repeat); and facility not maintained clean (repeat).

McDonald’s, 835 Lime City Road, Rossford, had three critical and four non-critical violations.

Critical were handwashing sink not accessible; chlorine sanitizing solution at incorrect temperature and/or concentration (repeat, corrected during inspection); and equipment food-contact surfaces or utensils are unclean.

Non-critical were no soap at handwashing sink(s) (corrected during inspection); non-food contact surfaces of equipment are unclean (repeat); mops dried improperly; and facility not maintained clean (repeat).

During a follow-up inspection, Reset and Cameos Pizza, 27250 Crossroads Pkwy., Rossford, had six non-critical violations, which were facility does not have an employee with manager certification in food protection; improper storage of food items; food employee(s) not wearing a hair restraint; equipment and utensils are not being air dried; non-food contact surfaces of equipment are unclean; and facility not maintained clean.

The following inspections were done March 14.

Dollar General Store, 30500 Drouillard Road, Walbridge, had four non-critical offenses, which were inadequate equipment available for cooling, heating or holding food; hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable; equipment components are not intact, tight or properly adjusted; and unapproved materials covering the walls and/or ceiling.

TA Operating, LLC Taco Bell/Pizza Hut, 12906 Deshler Road, North Baltimore, had one non-critical offense, which was non-food contact surfaces of equipment are unclean.

Speedway, 4661 Woodville Road, Northwood, had two non-critical offenses, which were non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair.

Love’s Travel Stops, 13190 Deshler Road, North Baltimore, had four critical and three non-critical violations.

Critical were equipment and utensils contacting temperature-controlled food not cleaned every four hours; improper cooling of temperature-controlled food; temperature-controlled foods not being hot held at the proper temperature; and temperature-controlled foods not being cold held at the proper temperature. All were corrected during inspection.

Non-critical were cutting blocks or boards cannot be effectively cleaned and sanitized; equipment components are not intact, tight or properly adjusted (repeat); and non-food contact surfaces of equipment are unclean.

Taco Bell, 2900 Woodville Road, Northwood, had one non-critical offense, which was facility does not have an employee with manager certification in food protection (repeat).

McDonald’s, 12776 Deshler Road, North Baltimore, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and floors, walls, and/or ceilings not smooth and easily cleanable.

Ten Pin Inc., 121 S. Main St., North Baltimore, had one non-critical offense, which was non-food contact surfaces of equipment are unclean.

Fuel Mart, 2185 Grant Road, North Baltimore, had one critical and one non-critical offense.

Critical was handwashing sink not accessible. It was corrected during inspection.

Non-critical was facility does not have an employee with manager certification in food protection.

Bibibop Asian Grill – French Quarter Square, 10630 Fremont Pike, Perrysburg, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being hot held at the proper temperature. It was corrected during inspection.

Non-critical were food employees wearing jewelry on arms or hands during food preparation; and improper storage of single-service and single-use articles. Both were corrected during inspection.

The following inspection was done March 15.

Cracker Barrel Old Country Store Inc., 27491 Helen Drive, Perrysburg, had four critical and three non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; food employee(s) did not wash hands when required; temperature-controlled foods not being hot held at the proper temperature (corrected during inspection); and insufficient air gap between the flood rim and the water supply inlet.

Non-critical were handwashing sink water below 100°F (corrected during inspection); non-food contact surfaces of equipment are unclean (repeat); and physical facilities not maintained in good repair.

The following inspections were done March 16.

Circle K, 28350 Oregon Road, Perrysburg, had four critical and six non-critical violations.

Critical were food employee(s) did not wash hands when required; quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (corrected during inspection); improper reheating of food for hot holding (corrected during inspection); and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked.

Non-critical were outer opening not protected; improper storage of food items; food employee(s) not wearing a hair restraint; non-food contact surfaces of equipment are unclean; cooking equipment or pan surfaces unclean; and areas designated for employees to eat, drink and use tobacco shall be located so that food, equipment, linens, single service articles, and single use articles are protected from contamination (corrected during inspection).

Taco Bell, 1130 S. Main St., Bowling Green, had two non-critical offenses, which were non-food contact surfaces not easily cleanable; and facility not maintained clean.

Fraternal Order of Eagles Aerie 2180 Inc., 1163 N. Main St., Bowling Green, had one non-critical offense, which was physical facilities not maintained in good repair (repeat).

Nagoya Japanese Steakhouse, 6190 Levis Commons Blvd., Perrysburg, had two critical and two non-critical offenses.

Critical were temperature-controlled foods not being cold held at the proper temperature; and ready-to-eat, temperature-controlled food not properly discarded when required. Both were repeat offenses, and both were corrected during inspection.

Non-critical were facility not maintained clean; and physical facilities not maintained in good repair (repeat).

Perrysburg Healthcare and Rehabilitation Center, 28546 Starbright Blvd., had one critical and three non-critical offenses.

Critical was working containers of poisonous or toxic materials not properly labeled. It was corrected during inspection.

Non-critical were facility does not have an employee with manager certification in food protection; improper storage of food items; and warewashing equipment and/or components not cleaned at required frequency.
