Cost for birth, death certificates increases

Beginning Oct. 16, the cost for a certified copy of a birth or death certificate will increase to $23.
The $4 increase is due to a "State Minimum Fee" increase required for vital statistics records
which was mandated by the state Ohio Department of Health (ODH) in order to enhance and maintain a
secure system that meets federal requirements.
The new fee distribution for a birth or death record is:
¥ $9.50 retained by the local health district issuing the certified copy
¥ $9 to the Ohio Department of Health
¥ $1.50 to the Ohio Family Violence Prevention Fund, Ohio Department of Public Safety
¥ $3 to the Ohio Children’s Trust Fund, Ohio Department of Job and Family Services
The increase was authorized by the ODH to allow the Electronic Death and Birth Registration System be
enhanced and to continue modernization efforts to move to a secure, paperless system from one
centralized database and maintain and upgrade infrastructure as needed.
Certified copies of birth or death certificates may be obtained from the Wood County Health Department at
1840 E. Gypsy Lane Road, Bowling Green. Information about how to request a copy can be found at
